This 76-year-old female from Vista, CA has multiple deformities following right mastectomy and TRAM flap breast reconstruction by another plastic surgeon. She had left breast cancer 30 years ago which was treated with mastectomy and implant reconstruction. The patient has significant breast asymmetry with a small right breast and abnormal shape. She has significant left breast scarring. There is a very unusual TRAM flap scar running across the middle of the abdomen through the belly button, instead of the usual location at the underwear line. There are also large abdominal hernias. Dr. Gupta performed multiple complex revision procedures, starting with complete reshaping of the TRAM flap. He reconstructed the fold underneath the breast and used a silicone implant to restore symmetrical breast size. He performed a left capsulectomy and implant replacement. Finally, he reconstructed the abdomen to repair the hernia and move the scar back down to the proper location.
